Newborn Health Shaped by Social and Environmental Inequalities
The health of babies from birth is deeply marked by the social and environmental conditions in which they grow up. In the United States, disparities in neonatal outcomes, such as premature births or low birth weights, are not random. They often stem from structural inequalities, such as past discriminatory policies, air pollution, or unequal access to healthy and safe neighborhoods.
These inequalities take effect long before birth. During pregnancy, chronic stress, poor nutrition, or exposure to pollutants can disrupt fetal development. The mother’s body, under stress, releases hormones like cortisol. Normally beneficial, these substances, in excess, can alter the functioning of the baby’s stress management system, with lasting consequences for their future health. The placenta, which usually protects the fetus, can have its role weakened by prolonged maternal stress, allowing more cortisol to pass through. This mechanism can program the child’s stress response system, making it more reactive throughout their life.
Epigenetic modifications offer another explanation for these lasting effects. Epigenetics studies how the environment can activate or deactivate certain genes without changing their structure. For example, prenatal stress can increase DNA methylation, a process that influences the activity of genes related to stress response. Studies show that babies exposed to high maternal stress during pregnancy exhibit epigenetic changes associated with an increased risk of metabolic problems or neurological development disorders.
The inequalities do not stop there. Historical policies, such as redlining—a practice that involved denying mortgages in certain neighborhoods based on their racial or ethnic composition—have created lasting disparities. These neighborhoods, often inhabited by minorities, have been deprived of investment for decades. Today, they still concentrate high levels of pollution, urban heat islands, and limited access to green spaces. Pregnant women there are more exposed to environmental risks, increasing the likelihood of premature births or low birth weight babies.
The consequences of these inequalities are also evident in access to prenatal care. Disadvantaged families, often facing food insecurity, unemployment, or precarious housing, have fewer resources to benefit from quality medical follow-up. These precarious conditions worsen the risks of complications during pregnancy and after birth.
Fortunately, solutions are emerging. Advances in exposomics—a discipline that studies the full range of an individual’s environmental exposures—now allow precise measurement of the pollutants or nutritional deficiencies to which fetuses and newborns are subjected. At the same time, local interventions, such as creating green spaces in disadvantaged neighborhoods or zoning reforms to move industrial areas away from residential zones, show promising results. For example, a 10% increase in urban tree canopy in previously disadvantaged neighborhoods has been associated with a 7% reduction in the risk of low birth weight.
Technological advancements, such as brain imaging or artificial intelligence, also reveal how social inequalities affect brain development from pregnancy onward. Babies from disadvantaged backgrounds sometimes exhibit differences in the structure or function of their brains, linked to prenatal exposure to stress or pollution. These gaps, observed from birth, can influence their future cognitive and emotional development.
To break this vicious cycle, an integrated approach is needed. This involves public policies targeting the root causes of inequality, such as renovating disadvantaged neighborhoods or improving access to healthcare. By combining these efforts with the latest scientific advancements, it becomes possible to give all children, regardless of their background, a healthier and more equitable start in life.
